When you arrive for your first visit, please be prepared to complete our new patient forms that will allow us to begin your dental treatment. For your convenience, our patient forms are available for you to download. Feel free to print and fill out the forms and bring them to your first appointment.
The dental record release form is only for patients who wish to have any recent dental records and x-rays transferred from the previous dentist. This form should be sent to your previous dental office with enough notice so that we might have your pertinent recent data such as x-rays at your appointment with us. By having these records at our office in advance of your appointment, you may avoid having to pay additional charges to duplicate relevant records.

If your first visit is for a dental emergency, only the tooth in question will be addressed. Depending on the diagnosis, an additional appointment may be necessary.

Payment options include cash, checks, Visa, MasterCard, Discover, American Express, CareCredit and any major or participating insurance.

We file your insurance as a courtesy to you and because we work with so many different insurance plans and have hundreds of separate procedures, it is impossible for us to know how much any insurance plan will actually pay for the dental service we perform. However, we can provide you with an estimated cost of the services being performed and what portion of that fee we expect your insurance will cover.
We will keep you informed of any fees not covered by your insurance, especially when non-routine services are anticipated. Ultimately, you will be responsible to pay all fees regardless of any insurance coverage.
